What CMS health and fire-safety inspection findings (deficiencies) mean, how we present them neutrally, and where to see them for local facilities.
During surveys, inspectors may cite deficiencies — specific findings measured against federal standards. We show them as factual regulatory records with their scope/severity, grouped into neutral harm bands. A cited finding is not proof of current conditions or wrongdoing, and missing data doesn’t mean zero.
